If the name on one or both forms of identification does not match the name listed in the vendor registration system (Pearson Vue), you must present an official document detailing why your name has changed. cards and photo bearing credit cards are not acceptable as a picture bearing identification. The second form of identification could be your passport or military identification card. The information on this card must match the name and address that you have provided to the Department of Business and Professional Regulation on your application for licensure.

One form of the identification must be a government issued card such as a driver's license or state identification card that also bears your picture and address. You must take two (2) forms of signature identification to gain admission to the testing site. The authorization letter confirms the examination for which the candidate has been approved by the Board Office, Pearson Vue Client Candidate Identification Number, and the telephone number to call for scheduling the examination. You do not have to wait for the letter to schedule your examination. Within 72 hours of receipt of the electronic file, Pearson Vue will email, if a valid email address is available, or mail the candidate an authorization letter. Once the appropriate Board office (DBPR) has approved a candidate's eligibility to take an examination, the candidate's file is sent electronically to Pearson Vue.
